My child and I are participants in the Music Together^®® program. I hereby give permission to Music Together and Music Together with Shannon, LLC to share my email and phone number with the other families strictly in the Music Together with Shannon classes that I attend. I understand that this can build community, allows for text/email threads and keeps me informed.

I also understand that, by enrolling in a Music Together® class at Music Together with Shannon, a licensed Music Together center, I agree that any information I provide will be shared with Music Together Worldwide (MTW). By signing this document, I represent and warrant that I am the parent or guardian of the child whose name appears next to mine in our account, and that we shall both be bounded by this agreement.
